Muskelaktivit\u00e4t entstehen schon in jungen Jahren und k\u00f6nnen effektiv mit Botox behandelt werden (Kr\u00e4henf\u00fc\u00dfe, Zornesfalten etc.). Falten, die durch den Verlust der Eigenelastizit\u00e4t der Haut entstehen, werden am Besten durch Straffungsoperationen (Facelifting etc.) korrigiert. Den Verlust von Unterhautfettgewebe kann man nur durch Einbringen von Eigenfett oder k\u00fcnstlichen F\u00fcllsubstanzen (\u201eFiller\u201c) wettmachen.<\/p>\n\n\n\n<h2 class=\"wp-block-heading\">Was sind Filler &amp; wie werden sie eingesetzt<\/h2>\n\n\n\n<p class=\"wp-block-paragraph\">Die erste k\u00fcnstliche F\u00fcllsubstanz war h\u00f6chstwahrscheinlich Paraffin, das bereits um 1900 eingesetzt wurde, um Substanzdefekte zu korrigieren. Aufgrund schwerwiegender Komplikationen wurde die Verwendung nat\u00fcrlich eingestellt. Um 1950 verwendete man tierisches Kollagen und Silikon\u00f6l. Bei Kollagen gab es h\u00e4ufig allergische Reaktionen (tierisches Eiwei\u00df), bei Silikon\u00f6l traten Unvertr\u00e4glichkeitsreaktionen auf. Um 1990 wurden die ersten Hyalurons\u00e4ure-Pr\u00e4parate auf den Markt gebracht. Hyalurons\u00e4ure ist ein Zuckermolek\u00fcl, das auch im menschlichen K\u00f6rper vorkommt und einige wichtige Eigenschaften besitzt. Hyalurons\u00e4ure bindet Wasser in der Haut (1000-fache Menge des Eigengewichts), baut ein mechanisch belastbares, dreidimensionales Netzwerk zwischen Zellen und Kollagenfasern auf und stimuliert die Bildung von Fibroblasten (Bindegewebszellen). Die F\u00e4higkeit Wasser zu binden verleiht der Hyalurons\u00e4ure ihre besonderen Flie\u00dfeigenschaften (\u201eViskoelastizit\u00e4t\u201c), weshalb sie sich zu Injektionszwecken besonders gut eignet.<\/p>\n\n\n\n<h2 class=\"wp-block-heading\"><strong>Eine funktionelle Einteilung der Filler ist in folgenden Kategorien m\u00f6glich:<\/strong><\/h2>\n\n\n\n<ul class=\"wp-block-list\">\n<li>\u201eg\u00e4nzlich abbaubar\u201c (resorbierbar), Abbau erfolgt innerhalb von 4-6 Monaten<\/li>\n\n\n\n<li>\u201everz\u00f6gert abbaubar\u201c (verz\u00f6gert resorbierbar), Abbau erfolgt innerhalb von einem Jahr<\/li>\n\n\n\n<li>\u201enicht abbaubar\u201c (nicht resorbierbar), wird vom K\u00f6rper nicht abgebaut<\/li>\n\n\n\n<li>Mischpr\u00e4parate<\/li>\n<\/ul>\n\n\n\n<p class=\"wp-block-paragraph\">Hyalurons\u00e4ure geh\u00f6rt zu den resorbierbaren Fillern die derzeit unter nachstehenden Produktnamen erh\u00e4ltlich sind: Hylaform, AcHyal, Belotero, Hyal2000, HylanSeS, Jolidermis, Juv\u00e9derm, Perlane, Puragen, Restylane, Revanesse, Rofilan Hylan Gel und Teosyal. Zu den resorbierbaren Fillern geh\u00f6ren nach wie vor Kollagenpr\u00e4parate wie Resoplast, Zyderm, Zyplast, Fibrel, Permacol, Evolence, Cymetra, Dermalogen, CosmoDerm, Cosmoplast, Autologen, oder Isologen.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Zu den verz\u00f6gert resorbierbaren Fillern geh\u00f6ren New Fill und Radiesse.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Zu den nicht resorbierbaren Fillern geh\u00f6ren Amazinggel, Aquamid, Argiform, Evolution, Outline, Bio-Alcamid, Adato Sil-ol 5000, Bioplastique, Dermagen, Silicex oder Silikon 1000.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Mischpr\u00e4parate aus resorbierbaren und nicht resorbierbaren sind Artecoll\/Artefill, Dermalive oder Dermadeep.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Permanente Filler bzw. verz\u00f6gert resorbierbare Substanzen bergen ein gewisses Restrisiko hinsichtlich Unvertr\u00e4glichkeitsreaktionen bzw. Kn\u00f6tchenbildung. Aus diesem Grund setze ich als k\u00fcnstlichen Filler lediglich Hyalurons\u00e4ure-Pr\u00e4parate ein oder transplantiere k\u00f6rpereigenes Fett.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Filler k\u00f6nnen in allen Gesichtsregionen eingesetzt werden, am h\u00e4ufigsten werden sie zur Vergr\u00f6\u00dferung der Lippen, zur Korrektur der Nasolabialfalten, zur Korrektur von Augenringen oder zur Aufpolsterung der Jochbeinregion eingesetzt.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Bei sachgem\u00e4\u00dfer Anwendung sind Filler sicher und nebenwirkungsarm, nach der Behandlung kommt es weder zu nennenswerten Schmerzen noch zu Ausfallszeiten. Dar\u00fcberhinaus f\u00fchrt die Therapie zu deutlich sichtbaren Verbesserungen bei minimalem zeitlichen und \u00fcberschaubarem finanziellen Aufwand. Bei konsequenter Anwendung \u00fcber Jahre w\u00e4re es \u00fcberlegenswert, als F\u00fcllsubstanz Eigenfett einzusetzen.
