A receding or protruding chin is frequently perceived as very bothersome, and many people also feel uncomfortable with a double chin. Fortunately, both the chin and double chin can be corrected with relatively minor surgical procedures. Small operations, implants, and fillers can help adjust the shape of the chin to better match the face, restoring harmonious proportions and boosting self-confidence. In some cases, an orthodontic procedure may be necessary. We also frequently perform chin correction in combination with rhinoplasty.

Chin Correction: Chin Shapes and Corresponding Corrections

Many people think that only the nose plays a central role in the face — but that’s not the case. The shape of the chin also plays a key role in facial harmony. The chin is important for facial proportions and strongly influences the overall appearance. Chin correction can significantly and permanently improve the facial profile. In combination with rhinoplasty, an adjusted chin interestingly makes the nose appear shorter. A corrected chin therefore affects not only the appearance of the chin itself but also the visual impression of the nose and neck.

In the harmonious proportions of the face, the length of the chin plays a crucial role. In an ideally proportioned face, the chin, upper/lower lips, and the foremost point of the forehead form one straight line. If the chin lies behind this line, it is referred to as a receding chin — this makes the nose appear larger and longer. If the chin, however, extends too far forward, it is called a protruding chin, which can give the face a harder or more dominant expression.

The Different Types of Chin Corrections

From a medical perspective, treatment is rarely necessary unless the cause is a jaw misalignment that leads to problems with swallowing, chewing, or speaking. If that is not the case, a chin correction is considered a cosmetic procedure — it refines the facial profile and enhances overall facial harmony.

There are three main types of chins for which we perform chin corrections in our clinic:

Overly Large / Protruding Chin

An unusually long chin is referred to as a protruding chin. Many people are bothered by a prominent chin because it stands out clearly in profile and often extends beyond the forehead and nose. This can make women appear more masculine. A large chin often gives the face striking, strong features and can make a person look stern or severe. People with this chin shape are often associated with traits such as courage, determination, willpower, and endurance — but also impulsiveness and impatience.

Sometimes, individuals with a very long chin have difficulty fully closing their mouth. The cause may be an overdeveloped lower jaw that is too long and large in comparison to the upper jaw. Conversely, the upper jaw may be underdeveloped, which can also make the lower jaw appear more prominent.

Correction options:

If the chin is too large and protruding but the teeth are correctly aligned and positioned, correction is almost always possible. In mild cases, the bone is simply shaved or contoured; in more severe cases, a portion of the bone may be cut and the chin tip repositioned. However, bone contouring alone is sufficient in most cases.

Receding Chin

A receding chin is a very common condition. Unlike a protruding chin, it appears too short and results from the backward position of the lower jaw. As a consequence, the nose and mouth seem more prominent, which is particularly noticeable in the profile view. A receding chin softens the facial contours — while this is primarily an aesthetic issue, it can significantly affect self-confidence, as the face may be perceived as less balanced or attractive. A surgical chin correction can help restore both harmony and self-esteem.

Correction options:

In mild to moderate cases — up to a projection deficit of about 10 to 12 mm — we perform chin augmentation using silicone implants. In more severe cases, an orthognathic (jaw) surgery may be necessary. Thanks to the variety of available chin implants, we can adapt the correction precisely to the patient’s facial structure — taking into account whether the lower jaw is broad or narrow, and whether a rounder or flatter chin tip is desired.

The silicone implants we use typically heal without complications and do not require replacement even after many years. Unlike breast implants, chin implants are never perceived as foreign bodies.

Double Chin

The correction of a double chin is by far the most common chin-related procedure. A double chin is not actually a deformity of the chin itself, but rather of the skin and soft tissue beneath it. The cause is almost always excess fatty tissue below the chin — often even in individuals of normal weight, as a hereditary predisposition frequently plays a role. However, a double chin can also develop due to loose or sagging skin, either age-related or following significant weight loss.

Correction options:

We correct a double chin either through liposuction or platysma tightening (tightening of the superficial neck muscle), and in many cases, both methods are combined. Only in extreme cases is a neck lift required to achieve the desired result.

It is not uncommon for patients to come to our clinic with what appears to be a double chin, but where the real cause lies not in excess fat or sagging skin — rather in a receding chin. In such cases, correcting the receding chin in combination with tightening the platysma (the neck’s skin muscle) or combining liposuction with skin tightening can significantly and sustainably improve both the neck contour and the overall facial profile.

This allows every patient to be well-prepared for our consultation, which typically lasts at least 30 minutes. During this session, we will discuss the surgical procedure in detail and address all individual questions. Even if you have already undergone chin correction elsewhere and are dissatisfied with the results, I am happy to provide a second opinion. However, please understand that in such cases, I charge a fee of 300 euros, which will be credited toward the cost of any corrective surgery performed by us.

Please refrain from smoking for at least one week before the procedure. If you are taking Marcoumar, please consult your treating physician about switching to another medication, as anticoagulant drugs can delay blood clotting. Medications containing acetylsalicylic acid, such as Aspirin, should also be discontinued 8 to 10 days prior to surgery. Please also avoid alcohol and sleeping pills as much as possible before the operation.

If the procedure is performed under local anesthesia, the following examinations should be carried out:

  • Complete preoperative blood count including blood type, HIV, and Hepatitis A, B, C

If the procedure is performed under general anesthesia or twilight sedation (sedoanalgesia), the following additional examinations are required depending on age:

  • Chest X-ray
  • ECG
  • Medical clearance from a general practitioner or internist

Please note that the examinations must not be older than 10 days at the time of surgery. Please bring all medical reports to the preoperative consultation.

Aspirin, Thromboass, and similar medications must not be taken within 14 days prior to the procedure. If necessary, they may be temporarily replaced with Heparin.

Surgical Procedure for Chin Correction

Depending on the extent and technique, the chin correction is performed under local anesthesia or twilight sedation (sedoanalgesia). The procedure takes approximately 1 to 2 hours. If the correction is carried out using implants, the surgical incision is made either behind the chin tip or in the fold of the lower lip. Through this opening, the implant is inserted, positioned, and fixed in place. If no platysma tightening is required, the procedure can be performed through the oral mucosa, leaving no external scar. In some cases, chin correction can also be achieved using autologous fat transfer.

Surgical Procedure for Chin Reduction

This is almost always done by bone contouring (burring). Afterward, a compression bandage must be applied to the chin to prevent reactive swelling.

Surgical Procedure for Chin Augmentation

If chin augmentation is desired and orthodontic surgery is neither necessary nor requested, a precisely selected silicone implant is inserted. The approach is usually made through the oral mucosa, leaving no external scar, though in some cases an external incision is used when a simultaneous platysma tightening is performed.

  • Plan the surgery date, measure and select the silicone chin implant.
  • Set the surgery date (take vacation or compensatory time off, arrange childcare if necessary, etc.).
  • Have the preoperative examinations (blood work, X-ray if necessary, ECG, surgical clearance) performed no earlier than 2 weeks before the operation.
  • Preoperative consultation on the day before surgery: findings are reviewed, final details discussed, and
  • the consent form signed. An admission form for the hospital will be provided by the assistant if required; please fill it out and bring it to the hospital. Otherwise, the procedure will take place in my surgical center. The first follow-up appointment will be scheduled in the clinic.
  • Arrive at the hospital 2.5 hours before surgery for admission formalities. Upon request, a staff member can accompany you and make the waiting time before surgery more comfortable.
  • Discharge usually takes place on the day of surgery. Please do not drive yourself; take a taxi or have someone pick you up.
  • Plan for rest at home and avoid physically demanding activities.

Aftercare:

After an internal chin correction, only liquid or soft foods should be consumed for the first few days. Avoid excessive use of facial muscles and the jaw, as well as intensive hair washing, hot showers, and physical exertion. Strenuous physical activities should also be avoided for about 4 to 6 weeks following the operation.

What is a chin correction?

A chin correction refers to either the shortening of a protruding chin or the lengthening of a receding chin.

How is a receding chin lengthened?

By inserting an anatomically shaped silicone implant, a receding chin can be lengthened — but only for a projection deficit of up to 12 mm. In more severe cases, orthodontic treatment is required.

How important is the chin for facial harmony?

The length and position of the chin play a decisive role in the harmonious appearance of the face. Ideally, the chin, lips, and the most prominent point of the forehead should form one vertical line.

How is a protruding chin shortened?

A protruding chin can be reduced through bone contouring (shaving). More severe cases must be treated orthodontically.

Can a chin correction be performed together with a rhinoplasty?

Yes, a chin correction can be combined with a nose correction. The overall surgical scope is only slightly increased. A chin correction usually takes no longer than 30 to 40 minutes.

What effect does correcting a receding chin have on a double chin?

Lengthening the chin naturally tightens the neck skin since this area is stretched. If further improvement of the neck contour is desired, a platysma tightening can be performed through the same incision. Liposuction — often requested by patients with a double chin — is not always sufficient or the method of choice.

What complications can occur after a chin correction?

Scars: Sometimes, if an external approach is necessary, visible scar formation may occur.

Infections and wound-healing issues: As with any surgery, there’s a risk of infection or delayed healing. With careful technique, however, these are very rare. The risk is higher among heavy smokers.

Bruising and swelling: Temporary bruising and swelling are common and usually subside within days or weeks.

Bleeding: Postoperative bleeding is rare and usually not serious but requires immediate medical care, as it may affect the final result.

Asymmetries and irregularities: These may occur if the implant shifts or is poorly sized but can be corrected through revision surgery.

Sensory disturbances: Minor nerves can be affected, leading to temporary numbness or tingling, which usually resolves on its own but may persist in rare cases.

Are silicone chin implants well tolerated?

Yes, silicone implants are generally well tolerated and integrate smoothly into the tissue without being perceived as foreign objects.

Where do scars remain after chin surgery?

If the incision is made externally, a 3–4 cm scar remains under the chin. It may appear red for several months but fades over time. If performed intraorally, the incision is hidden in the mucosal fold of the lower front teeth — invisible from the outside.

When can I resume normal activities after chin surgery?

The surgical area usually heals within 10–12 days. Swelling can last 3–4 weeks, but after 4–6 weeks, the area looks completely normal again. Light physical activity is possible after 2 days, while social and professional activities can typically resume after 10–12 days.

What are the long-term results of a chin correction?

The long-term results are generally excellent. In rare cases, minimal bone resorption under the implant may occur over the years, slightly reducing chin projection.

Can I conceal a receding chin without surgery?

If the chin is severely receding, concealing it can be difficult. However, contouring makeup or emphasizing the cheeks and eyes can improve facial aesthetics. Hairstyles that frame the face — such as a chin-length bob — can also help. Men can grow a beard to visually balance a receding chin. Still, for lasting improvement, surgery is typically the only effective option.
