Botox works by blocking nerve signals to the muscle. The muscle contracts less, and the skin above it no longer forms such deep wrinkles. Botox is not a filler; it does not “plump” the skin.

For wrinkles caused primarily by a loss of volume, fillers or autologous fat are generally the preferred options.

What are realistic expectations?

When used properly, Botox is considered a safe procedure with few side effects and no significant downtime. Temporary swelling, minor bruising, or tiny scabs at the injection sites may occur.

In rare cases, there may be a temporary effect on nearby muscles. Because very fine needles are used, the treatment is generally relatively painless.

What is Botox?

Botulinum toxin (BTX) is a naturally occurring protein produced and released by various strains of the bacterium Clostridium botulinum in the absence of air. As early as the mid-18th century, poisonings accompanied by paralysis were observed after the consumption of spoiled sausages. In 1817, speculation arose as to whether this still-unknown toxin could be used to treat muscle spasms. Finally, in 1897, it was discovered that the symptoms were caused by the consumption of a protein produced by a specific bacterium in the absence of air.

How does Botox work?

Botox blocks the neurotransmitter acetylcholine (Ach). This prevents the transmission of nerve impulses to the muscle, which can then no longer contract. The elimination of facial wrinkles with Botox is therefore achieved by relaxing the muscles, preventing the overlying skin from forming wrinkles. Botox is not a filler; therefore, wrinkles are not “filled in” with Botox.

How long do the effects of Botox last?

Botox takes effect within 24 hours and is broken down by the body within 3 to 6 months, at which point it loses its effectiveness. For long-lasting results, Botox must therefore be administered at regular intervals.

Does Botox work for everyone?

No, 3–7% of the population are immune to Botox or show only a mild reaction.

What is Botox used for?

Botox is particularly suitable for treating the glabellar region (“frown lines”), the forehead (“worry lines”), and “crow’s feet” (smile lines at the corners of the eyes). Experienced physicians can also use very small amounts of BTX to lift the eyebrows (“chemical brow lift”), widen and round the eyes, reduce radial wrinkles on the upper lip, and lift drooping corners of the mouth.

Can Botox also be used to treat excessive sweating (hyperhidrosis)?

Yes, Botox is used to treat hyperhidrosis in the underarm, hand, and foot areas. This is possible because the activity of sweat glands is controlled by nerve impulses, and Botox can reduce the rate of these impulses by blocking acetylcholine (ACh). This results in a significant improvement in quality of life for those affected. Unlike other (surgical) procedures to suppress sweat production, BTX therapy does not cause compensatory sweating in other areas of the body.

Is Botox treatment painful?

The injection needle is very thin, so the procedure is relatively painless.

What complications can occur?

When administered correctly by an experienced practitioner, the use of BTX is a safe procedure with few side effects. Nevertheless, as with any medical treatment, side effects and complications can never be completely ruled out. Non-specific side effects include swelling, minor bruising, and tiny scabs at the injection sites. Occasionally, temporary weakness or paralysis of adjacent muscles may occur.

Can a Botox treatment be done during my lunch break?

Yes, absolutely. A Botox treatment typically takes between 5 and 10 minutes. The injection sites can be covered with makeup after the treatment.

Which medications containing the active ingredient botulinum toxin are approved for use?

In Austria, botulinum toxin type A is offered as a medication by four different companies: “Botox” and “Vistabel” from Allergan, “Dysport” from Ipsen, “Xeomin” from Merz, and “Neurobloc” from Elan Pharma. All of these preparations contain botulinum toxin type A, but differ in their formulation and concentration. Similar to “Aspirin,” whose brand name is used to refer to medications containing the active ingredient acetylsalicylic acid, the brand name “Botox” is used for a range of pharmaceutical products containing botulinum toxin.
