Breast augmentation using the patient’s own fat allows for a natural enlargement without implants and combines it with body contouring through fat removal.


The procedure is performed in a single session and consists of two distinct steps. First, fat tissue is removed using a gentle liposuction technique. The fat cells are then processed and carefully injected into the breast to enhance its shape and volume. There are few, if any, visible scars.

How does the procedure work?

    Fat removal

    The harvested fat is processed, separated from blood and fluid, and enriched with PRP.

    Achieving lasting, natural-looking results requires experience and a gentle technique to ensure that as many fat cells as possible survive.

    The processed fat is injected into the appropriate layers of breast tissue through tiny incisions using a thin cannula, not into the glandular tissue.

When is breast augmentation using autologous fat a viable option?

Requirements for the procedure

Physical Requirements

Sufficient donor fat must be available for the procedure; usually, a normal BMI or even slightly below is sufficient, as enough fat can often be harvested from the abdomen, hips, thighs, or upper arms. It is very rare that no fat can be harvested at all.

It is not possible to predict exactly in advance how much fat will permanently take, as this varies from person to person and depends, among other things, on breast size and tissue quality. Softer or already stretched tissue, such as after pregnancy and breastfeeding or after implant removal, often absorbs fat better.

For very small or firm breasts, pre-stretching may be advisable. This is done over several weeks using a vacuum cup and can improve the survival rate.

Medical Requirements

In addition to having sufficient fat tissue and a stable body weight, the patient must, of course, be in good health for the procedure to be performed. Before breast augmentation using autologous fat, we recommend a mammogram and an ultrasound examination—both to rule out breast cancer and to have a baseline image for future imaging.

Important Note

Women who smoke are strongly advised to quit smoking if they are considering breast augmentation using their own fat, as nicotine causes blood vessels to constrict. Good blood circulation is essential for the fat tissue to heal properly.

Can breast augmentation using autologous fat replace breast augmentation with implants?

Breast augmentation using the patient’s own fat tissue can already produce remarkable results. Of course, there are limits to the procedure, which depend on the anatomical characteristics of the breast (size, firmness, prior stretching, etc.). From a medical standpoint, there is no upper limit to the amount of fat that can be injected, as long as the breast tissue remains soft and supple after the fat injection and the fat is injected in multiple layers. Generally, however, 350 ml of transplanted fat is usually the upper limit for the first surgery if the patient has not previously been pregnant or had an implant removed. To inject 700 ml of fat, approximately 1000–1200 ml of fat must be harvested; however, not every woman has this amount of fat available. If the woman is very slim and fat must be conserved, it is recommended to proceed in small stages.

What is the minimum age requirement for having breast augmentation surgery using my own fat?

The patient should be fully grown before undergoing the procedure. Patients aged 18 and older are eligible for the treatment.

How do weight fluctuations affect my breast after surgery?

After breast augmentation using your own fat, you should avoid weight fluctuations of more than 5 kg. If you lose weight, the augmented breast may naturally become flatter; if you gain weight, the skin will stretch, but this is only a problem with significant weight gain. When you lose or gain weight, the supporting tissue in the breast is stretched. This can cause the shape of the breast to change.

Does the body break down the transplanted fat after breast augmentation?

No, once the fat has been absorbed into the breast tissue, it does not break down any further. The results are permanent. The absorption rate in the breast ranges from 40% to 80%. This means that 2–3 procedures are typically required to achieve the desired result, although some patients are satisfied after just one surgery.

What results can be achieved with autologous fat in breast augmentation?

With this method, the breasts can be enlarged by a maximum of 2–3 cup sizes, depending on the patient’s anatomy and the amount of fat available.

Can complications arise during breast augmentation using autologous fat grafting?

Complications are rare with this procedure. Aside from the usual general surgical risks, such as infection (extremely rare), only mild bruising and swelling may occasionally occur, but these subside over time. Occasionally, unevenness may occur around the harvest sites, or minor asymmetries in the breasts may remain; both can be easily corrected. Since autologous fat heals at varying rates, it is impossible to predict how much fat will remain. The final result can be assessed after three months.

Can scars remain after breast augmentation using autologous fat?

During this procedure, only very small incisions—between 2 and 4 mm in length—are made. Essentially, they heal almost without a trace after the operation. Because the incisions are so small, no stitches are needed. As a result, there is hardly any scarring. If any scars do remain, they are barely noticeable.

How can I reduce swelling in my breasts after the procedure?

After surgery, patients should preferably sleep on their backs and use a raised headrest. Since good blood circulation is necessary for the fat cells to take hold, the area should not be cooled. During the day, limit physical activity and avoid sports for two weeks.

When is it possible to undergo another breast augmentation using autologous fat?

Breast augmentation using autologous fat can be repeated after 3 months.

Can I sleep on my side or stomach after the procedure?

After breast augmentation using autologous fat, you should avoid sleeping on your side or on your stomach. The most comfortable position is to sleep with the head of the bed elevated.

When can I start lifting heavy objects after breast augmentation with autologous fat?

Lifting heavy objects is generally not a problem after two weeks, but you must not put any pressure on your chest. You should avoid saunas for at least four weeks, refrain from all sports for two weeks, and avoid high-impact activities (such as running and aerobics) for eight weeks.
