Eyelid Surgery – More Than Just Removing Fat

Eye bags are not caused by tears. They are fat deposits that normally lie beneath the eyeball and protrude forward due to weakened connective tissue.

Dark circles are often present as well. In such cases, the fat must not simply be removed—otherwise, the result will be a hollow, sunken appearance. The goal is a harmonious distribution of volume, not a hollowed-out look.

For over 25 years, I have been correcting under-eye bags using a variety of techniques—often in combination with volume restoration through autologous fat grafting—to achieve a natural, long-lasting result.

Causes and Symptoms of Under-Eye Bags

Eye bags are located below the eyelids, on either side of the nose, and appear as bulging swellings. They consist of fat deposits that slip forward due to a weakening of the supporting structures.


They can appear at a young age. In younger people, they usually occur without dark circles. Starting around age 35–40, under-eye bags and dark circles often appear together.

Misdiagnosis: Swelling or Lymphatic Congestion

Not every instance of morning puffiness is a bag under the eye. If the swelling subsides as the day goes on, it is usually caused by fluid retention or impaired lymphatic drainage—not actual bags under the eyes.

How do you treat under-eye bags?

Eye bags without dark circles (common in young patients)

The protruding fat is either removed or repositioned, and the supporting structures are reinforced. The procedure is performed via a transconjunctival approach (through the inner surface of the lower eyelid)—leaving no visible scar.

To avoid unevenness and maintain a youthful volume, I often combine this procedure with an autologous fat graft. The autologous fat is enriched with PRP to improve healing and malleability.

Eye bags and dark circles

In these cases, the fat must not be removed. Instead, it is mobilized and repositioned downward over the bony rim of the eye socket (fat repositioning). This simultaneously fills in dark circles under the eyes.

Since the volume is not always sufficient, I regularly combine the procedure with additional autologous fat grafting to even out differences in contour and achieve a smooth, natural lower eyelid contour.
