Concern over the loss of attractive volume leaves many women afraid to take this step despite increasing complaints.

Many of these patients may now be helped: in a single operation, the implants can be removed, and the same volume can be restored by a multilayered autologous fat transfer. Because the implants have pre-stretched the breast tissue the fabric can absorb the fat optimally. A supplemental transfer is of course possible after a short time (2-3 months).

Assuming that patients have about 1 liter of fat that can be used for the purpose of autologous fat transfer, the operation ‘silicone out / autologous fat in’ is an excellent solution to this long-term complication of breast augmentation.

Besides using autologous fat for breast augmentation when silicone is undesirable, more recently, there are increasing requests for a change: i.e. Silicone out, fat in.

The discovery of stem cells in adipose tissue ( 2001) opened up previously unimagined possibilities for autologous fat transplantation. The stem cells of the fatty tissue are called ADSC (adipose derived stem cells). They differ from the fat cells mainly through the valuable property of being able to evolve in response to their environment into different cell types (niche theory) . These include, inter alia, Skin tissue, connective tissue, blood cells, blood vessels and fat cells. Quickly, highly respected university research groups were established worldwide, with the aim of exploring the possibilities of ADSC and improving surgical results.

The integration rate of transferred autologous fat varies btw. 30-80 %, therefore multiple sessions may be required to achieve the desired effect. Operating expenses of autologous fat transfer and the related costs appear towards the introduction of an artificial filler (hyaluronic acid , etc.), although higher in the long run the autologous fat transfer is however cheaper and less time-consuming. Artificial fillers are lost / reduced within 6 months and must be subsequently injected at regular intervals.

With special cannulas (small diameter, small holes) fat is particularly gentle aspirated from a body and introduced after appropriate treatment in the desired recipient region. The top priority here is to avoid overpressure (prevents blood flow) and a multi-input technology, so each ‘fat spaghetti’ is surrounded by well-perfused tissue so the take rate can be optimized. Other measures to improve the take rate include pre-stretching the recipient’s bed with vacuum (chest), the accumulation of the introduced fat with stem cells of the fatty tissue (only works if fat ‘abundance’ is present, so if more than double the planned filling volume for removal is available) and the addition of PRP (growth factors from centrifuged blood).

In the aesthetic field autologous fat transplantation is used for natural facial and breast augmentation. In the face, autologous fat correction is used for age-related loss of subcutaneous fat and is, in addition to the face-lift, the best procedure for facial rejuvenation. Specifically, it is used for lip augmentation, for the correction of nasolabial folds, the dark circles, plumping sunken cheeks, correcting upper eye lid creases and sunken temples.

For breast enlargement own fat is also eminently suitable. Here attention to the preparation of the breast (vacuum pre-strain) should be taken and the patient should be stated that often has to be reckoned with 2-3 sessions.

In reparative / reconstructive areas, autologous fat transplantation can correct dents or scars (after OPs, etc,…) and some breasts can be corrected or reconstructed after breast cancer. Also mistakenly removed puffiness and hence the dark circles can be corrected with autologous fat.

Since 1990, the triumph of autologous fat transplantation is unstoppable. The method of surgery is increasingly used both in aesthetic and in reconstructive areas, because the own fat simply is the best filler and because it also has significant reparative and reconstructive properties due to the presence in the fat stem cells. Here, the process is technically simple and used for Pat. Hardly onerous (outpatient / day clinic), but the surgeon must have extensive experience in order to achieve a satisfactory take rate after autologous fat transplantation.

Transferred fat is not broken down and the result obtained is thus permanent.
